Arthoscopic Partial Meniscectomy in the Meniscal Injuries



Abstract
Arthroscopic surgery in the meniscal injuries is commonly used, because there are advantges such as less injury to the joint, almost absence of postoperative pain, rapid and complete recovery of the joint motion without physiotherapy and economic
benefits due to shorter periods of admission.
We studied the clinical values according toe the type and site in the 80 meniscal tears()77 knees0 which were treated by arthroscopic partial meniscectomy in Kyung Hee Medical Center from jun. 1989 to Dec. 1993. Monisci, which had undergone the
arthroscopic total meniscectomy or arthroscopic meniscal repair, or had discoid meniscal tear or associated complete ACL tear, were excluded. The duraiton of follow-up was from 1 year 5 months to 6 years 2 months(average; 2 years 3 months), the age of
the patients ranged from 17 years to 63 years(average; 29.0 years old0 and the majority of the patients was under 30 years old as 46 cases959.7%). The 66 patient had no specific trauma history. Among the 80 cases, medial meniscal tears wee douns in 36 cases, llteral meniscal tears in 38 cases, both meniscal tears in 3 cases. As for the distribution according to the type of the meniscal tears, there were 21 cases in bucket-handle type, 18 cases in longitudinal tear, 13 cases in horizontal tear and 12 cases in flap tear.
To record the clinical results of arthroscopic partial meniscoectomy, we used the modified Marshall knee scoring scale and Lysholm knee scoring scale. The preoperative and postoperative average knee score were as follows; 31.8 points and 42.8
points of the modified Marshall knee scoring scale; 53.8 points and 85.9 points of the Lysholm knee scoring scale. The knee scores of follow-up were high in patients who had longitudinal and bucket-handle tears; but relatively low in ones who had
degenerative and horizontal tears. The postoperative results were better in the lesions at the anterior and middle horn than those at the posterior horn. Reoperation was done in 2 cases of degenerative tears because pain and effusion had occured after
arthroscopic partial meniscectomy.
